Is there a template the would utilize the source citation that is at the bottom of the find a grave memorial page for an individual?

If you are satisfied that it contains the desired information in an acceptable format, just paste it into a Free Form. There's no point parsing it into bits and pieces only to reassemble them through a template.

Many if not most genealogy database sites these days provide you with a completed footnote sentence that is fairly comparable to what is provided by Find A Grave. Tom's sage advice is equally applicable to any such site.
