Citing online death certificate images

I have the online images of death certificates from Ancestry for quite a few of my ancestors. When I look at the source templates, I only find death certificate, local or state level, or death registration, local level. If I use the state level, do I put the State (Texas in this case) under Agency/Creator, Ancestry as the repository, and http://www.ancestry.comas the repository location? If I do that then Ancestry is not italicized and the URL is not in parenthesis - nor does it have a place for date accessed. Am I looking at this all wrong? Is there another way to do this?

I use the source template "Military Records, Databases" for the majority of databases I find on Ancestry and other sites. One master source per database. I can usually fit everything I want to note about the database and the record I found there using it. Sourcing is so individual there really isn't any right or wrong answer, as long as people can find it again.
